Please read below as it includes important information on how to start the process. During phase one we are distributing the vaccines to sections A1 and sections 1B. In order for this to be a fair process for everyone please only submit your information once.

One of our representatives will contact you to schedule an online consultation with a healthcare provider to see if you qualify for the vaccine. The Provider will also review your medical history and discuss the risks and benefits of the Moderna vaccine.
If you meet the requirements, an appointment will be made for you to come in and receive your vaccine dose.

The Covid-19 Vaccine – Coming Soon to an Urgent Care Near You!
As of 12/11/20, the FDA has provided an EUA approval (Emergency Use Authorization) to the Pfizer and BioTech’s COVID-19 Vaccine. This is one of five coronavirus vaccines that are under review by the FDA.
Five drug companies have versions in Phase III clinical trials. They are up for FDA review and could become available within 1-2 months if quickly approved. Five different companies are seeking an EUA (Emergency Use Authorization) to get the vaccines approved without the usual waiting period.
There are currently five coronavirus vaccines in progress:
Pfizer/BioNTech’s COVID-19 vaccine
It’s an RNA-based vaccine claiming 95% effectiveness in preventing COVID-19. For treatment, you’ll require two doses given several weeks apart. Pfizer was the first to announce success in developing this experimental kind of vaccination.
AstraZeneca’s COVID-19 vaccine
An altered virus-based coronavirus vaccine claiming to be 90% effective against COVID-19. Like most of the others, you will need two doses 30 days apart.
Janssen’s COVID-19 vaccine
Janssen is a subsidiary of Johnson & Johnson. It started large-scale COVID-19 trials in Chicago on Dec 8th. Unlike the other COVID-19 vaccines, it is a single-dose treatment. Learn more about their vaccine study and research study on their website.
Moderna’s COVID-19 vaccine
This is an RNA-based modified virus vaccine. The second drug company to announce a successful coronavirus. Moderna claims it is 95% effective at preventing COVID-19.
Novavax’s COVID-19 vaccine
This protein antigen-based COVID-19 vaccine saw some delays but is now entering late-phase trials in the United States. Phase III enrollment finished at the end of November, and testing is happening right now.
