Xpress Urgent Care can perform USCIS Immigration Physicals in accordance with the requirements of the Department of Homeland Security (DHS) and U.S. Citizen and Immigration Services (USCIS) for immigrants from another country. Each evaluation includes an examination performed according to the medical guidelines established by the DHS, USCIS, and the Centers for Disease Control (CDC). Our examining civil surgeon will record your exam results on Form I-693 for you to return to the USCIS. If you’re seeking Permanent Residency or a Green Card, and are required to obtain an immigration physical prior to the approval of your application, schedule your immigration physical today.
Included in the Immigration Physical examination:
- Physical examination and completion of all the necessary paperwork
- Physical and mental evaluation
- A review of all available medical records
- Evaluation of other physical or mental abnormality, disease or disability
- Vaccinations and status verification
- Chest x-ray (if needed)
Administration of vaccines and any other required services are available at additional cost.
What you MUST bring to your USCIS Medical Exam
- Government-issued picture ID
- Vaccine/Immunization records
- Proof of COVID vaccination must be brought to appointment
- Patients must be fully vaccinated before the form is released
- Xpress Urgent Care does not provide COVID-19 vaccinations
- Hospital or clinic records, including proof of prior treatment for tuberculosis or other infectious disease
- Prior chest x-rays and/or other diagnostic tests
- Any mental health records

What you MUST bring
Patients are advised to bring the following to their appointment:
- Government issued picture ID
- Vaccine/Immunization records
- Proof of covid vaccination
- Hospital or clinic records, including proof of prior treatment for tuberculosis or other infectious diseases.
- Prior chest x-rays and/or other diagnostic tests
- Any mental health records
- Payment (we accept debit, and credit card)
- Translator, if you are not fluent in English and will require another individual’s help in answering questions.
USCIS Immigration Medical Exam Process
First Visit: Information for your initial visit:
The USCIS immigration medical exam process typically takes a total of two visits to complete. When you are ready to begin the exam process you can do so by walking into any Xpress Urgent Care location. We accept patients on a walk-in basis 7 days a week from 8 am to 8 pm. Your initial visit allows us to review your paperwork and get started by running some tests.
The staff will also review your immunization status and inform you of any outstanding vaccines. Please bring any necessary documentation as it will be recorded on your I-693 form.
Second Visit: Information for your follow up visit:
Your follow-up visit involves being seen by our civil surgeon who will review your laboratory results and conduct a physical. The civil surgeon will also ensure that you have satisfied all the immunizations as required by the I-693 process. If there are any requirements for outside consultation the civil surgeon will notify you during the second visit. Typically, your follow-up visit will occur 3-5 days after your initial visit.

After the Exam
The Civil Surgeon or one of the assistants will place the original I-693 form along with supporting documents in a sealed envelope. An exact copy of the sealed i-693 will also be provided to the applicant.
